The Figueroa Wu Family Foundation (FWFF) is dedicated to creating a safe environment for all of our volunteers. As part of this commitment, we ask all volunteers to read and sign the following Waiver and Release of Liability Agreement before participating in any FWFF programs, including but not limited to the Pilsen Food Pantry and El Mercadito Gratis.

Important Information:

• The Volunteer Waiver Form needs to be completed only once every 5 years

• Volunteers (excluding their legal guardians or designees) are required to complete the Volunteer Waiver Form linked below
